Landlord Approval Form

Thank you for considering TeamPharr.Net as your internet service provider. To ensure you have a smooth
installation experience, if you don’t own the property, we ask that you get your landlord’s approval first
before proceeding with the first month’s payment and installation.

We may have to make modifications to the property during installation. These modifications are often
minor, like drilling holes in order to run cable and attach equipment securely to the outside of the
structure. Any kind of modification may not be allowed based on the terms of your lease/rental agreement
or may require pre-approval by the landlord. Landlord approval is often required with multi-tenant
apartment buildings and rental homes.

You should carefully review your lease agreement to determine if you have the right to make these
modifications. If landlord approval is required, please request that the landlord, or its authorized
representative, complete the section below to acknowledge authorization for the installation and provide
this signed form to TeamPharr.Net by emailing us at support@teampharr.net, by mail or in person at 100
W. Ferguson Ave, Pharr, TX 78577.

By providing this signed form to us, you’re giving us permission to install your new TeamPharr.Net Internet
service and receiving equipment, and are releasing us from obligation to remove your equipment after
we install it. This signed installation permission form releases us, TeamPharr.Net (including any
installation technician), from any liability related to damages your landlord may claim as a result of an
alleged violation of your lease/rental agreement as it relates to the installation of your new
TeamPharr.Net service.

Should your landlord only approve our service installation subject to special requirements that make the
actual installation more expensive (or exceeds what we offer as part of our standard professional installation),
you will be asked to pay a corresponding charge, to be agreed upon by you and your installer, for the
additional services.
